About Susan Klotz FNP
Susan Klotz was born in St Charles Missouri and was raised in the Ozarks with her 2 older brothers. Susan and her husband raised 3 boys in Willard Missouri, and now reside on their hobby farm in Ash Grove Missouri.

Susan enjoys spending time with her grandchildren, working on her farm, fishing, and kayaking. Her and her husband are known for taking in strays and rescues in need of care. She also volunteers at Camp Barnabas, a local camp for children and young adults with special needs.

Susan graduated from Glendale High School in Springfield Missouri in 1996. She went on to pursue her nursing degree with Cox College, and eventually graduated with her Masters in Nursing Science in 2011. She became board certified in family practice through the ANCC in 2012, and began practicing at a free clinic in Springfield MO, where she remained for 3 years. She then hired on with a physiatry group in Springfield Mo, specializing in Physical Medicine and Rehabilitation managing spasticity and function. In May 2020, Susan started Your Health Consultant, an independent practice in Ash Grove Mo.

Susan specializes in functional medicine including intrathecal baclofen pump management and mobility care and manages conditions including cerebral palsy, MS, spinal cord injuries, brain injuries, and amputations. She is also providing primary care, sports physicals, and urgent care.

Our clinic is located in Ash Grove, 25 minutes north of Springfield Missouri, and conveniently located right off of Hwy 160. House Calls available for acute needs and intrathecal baclofen pump management with insurance approval. Virtual visits are also available for established patients.

Your Health Consultant provides personalized care to individuals with cerebral palsy, multiple sclerosis, spinal cord Injuries, brain Injuries and other functional deficits and mobility concerns that affect quality of life. She specializes in spasticity management including therapy recommendations, adaptive equipment recommendations and Intrathecal baclofen pump management.

To meet the needs in the community, Your Health Consultant is now providing Primary care, sports physicals, and urgent care needs.

In accordance with the Missouri APRN guidelines, Susan has a collaborative practice agreement with Dr. Jesse Neeley DO.
